The Idaho Department of Insurance is working on going paperless. We have made the required Health Forms, the Health Survey, and TPA Health Survey electronic in the hopes of making the process easier for Carriers and TPA’s. If you are a Carrier please complete the Health Forms and the Health Survey. If you are a TPA please complete the TPA Health Survey.
