1. Type of document (such as a Certificate of Compliance or Deposit)
2. Number of documents
3. Company name
4. Company's NAIC number (not required)
5. Date required

Fees: Per Idaho Department of Insurance Rules, IDAPA 18.01.44.011, the fee must be paid in advance by check payable to the Department of Insurance.

• $20 per certification (IDAPA 18.01.44.040.03)
• $50 per certification for a certified copy of Certificate of Authority (IDAPA 18.01.44.040.01)
