Change of Control

Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA). Please complete the Corporate Amendments Application found at the National Association of Insurance Commissioners (NAIC) Web site.

1. No prior notification is required if the acquisition is beyond the scope of Idaho Code §41-3805B(2). Form E thirty (30) day notice required otherwise.
   
   
2. Please submit a post acquisition notification within 180 days and include the following information:

   1. Application Form and Attachments (UCAA Form 1C & 2C)
   2. Filing Fee (none)
   3. Articles of Incorporation Bylaws *
   4. Plan of Operation *
   5. State-Specific Information ( Information Statement )
   6. Uniform Consent to Service of Process (UCAA Form 12)
   7. State of Domicile Approval ( a copy of the order or the approval)
   8. NAIC Biographical Affidavits (not required)

      * Submit only if changed as a result of the Change of Control

Please indicate the effective date of the Change of Control in the state of domicile in your cover letter and submit all the documents to:
