Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA). Please complete the Corporate Amendments Application found at the National Association of Insurance Commissioners (NAIC) Web site.

   Completet the Information Statement form online.

Note: Although not listed in the Table of Contents for the UCAA Section III, a certified copy of a Certificate of Compliance reflecting a new company name issued by the state of domicile must accompany the UCAA Form 2C.

Fees:  There is no fee for these filings.

Please indicate the effective date of the Name Change in the state of domicile in your return correspondence and submit all the documents to:
