Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA). Please complete the Corporate Amendments Application found at the National Association of Insurance Commissioners (NAIC ) Web site.

   2. A certified copy of Certificate of Deposit. (Click to see Idaho's minimum requirements.)
      Note: Although not listed in the Table of Contents for the UCAA Section IV, a certified copy of a Certificate of Compliance issued by the new state of domicile must accompany the UCAA Form 2C.

Fees: There is no fee for these filings.

Please indicate the effective date of the Redomestication in the state of domicile in your return correspondence and submit all the documents to:
