Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA). Please complete the Corporate Amendments Application found at the National Association of Insurance Commissioners (NAIC ) Web site.

UCAA Section IV - Redomestication:

1. Application Form and Attachments (UCAA Form 1C & 2C)
2. Filing Fee (none)
3. Articles of Incorporation (a certified copy issued by the new state of domicile)
4. Bylaws (a certified copy if amended)
5. Service of Process (UCAA Form 12)
6. State of Domicile Approval (a copy of the approval from the new state)
7. State Specific Information
   1. A certified copy of Certificate of Compliance issued by the new state reflecting the lines of business authorized in the state)
   2. A certified copy of Certificate of Deposit issued by the new state ( Idaho's minimum requirements .)
   3. Information Statement
   4. Original Idaho Certificate of Authority

Please indicate the Effective Date of the Redomestication in the state of domicile in your cover letter and submit all the documents to:
