Voluntary Withdrawal of Certificate of Authority
Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA ).
The following documents are required to process a withdrawal of the Idaho Certificate of Authority.
Submit:
- A letter requesting to withdraw from doing business in Idaho indicating the following:
- Brief explanation of the reason (e.g., Merger into the company not license in Idaho)
- Whether there are any Idaho policyholders and if yes, provide the company name and the NAIC Company Code of the assuming company.
- Original Certificate of Authority.
Note: If you cannot locate the original Certificate of Authority, please submit the Affidavit - Loss of Certificate of Authority certified by a company's authorized officer with notary's signature.
Fees: There is no fee for this filing.
