Voluntary Withdrawal of Certificate of Authority

Idaho is a Uniform State, which accepts the Uniform Certificate of Authority Application (UCAA ).

The following documents are required to process a withdrawal of the Idaho Certificate of Authority.

Submit:

  1. A letter requesting to withdraw from doing business in Idaho indicating the following:
    1. Brief explanation of the reason (e.g., Merger into the company not license in Idaho)
    2. Whether there are any Idaho policyholders and if yes, provide the company name and the NAIC Company Code of the assuming company.

  2. Original Certificate of Authority.

Note:  If you cannot locate the original Certificate of Authority, please submit the Affidavit - Loss of Certificate of Authority certified by a company's authorized officer with notary's signature.

Fees:  There is no fee for this filing.
