How to File a Request for Consumer Assistance
You may call the Department of Insurance and speak to a Consumer Affairs Officer about your problem.

The Consumer Affairs Officer will assist you with your problem. If it appears that your problem cannot be resolved over the phone, then you will be asked to request our assistance in writing.
You may either:
- Write us a letter
- Complete our consumer complaint report
- File a consumer complaint on-line
Please give us as much information as you can about the situation. For example, policy numbers, agent name, company name, details of the situation, etc. The more information we have the faster we will be able to assist you.
