Idaho Resident Business Entity License Paper Application Checklist

- NAIC Uniform Application for Business Entity Insurance License/Registration
- Use the Add Lines Request form if adding a line to an existing license as Idaho does not charge a fee to add a line. This transaction will require listing a registered producer who carries the line of authority requested.
Financial Requirements
- Check or money order in the amount of $80, made payable to Idaho Department of Insurance for: Producer license/MGA/Independent Adjuster/Public Adjuster
- Title license: $50
- TPA license: $300.
Other Requirements
- Officer of the firm must sign and date application
- Business must be registered with the Secretary of State and must be incorporated as one of the following entity types: corporation, limited liability partnership, limited liability company, partnership.
- At least one licensed producer must be registered to business license and should be listed on application where appropriate. More than one producer may be listed if necessary to cover lines of authority.
- Attach a check or money order for appropriate fee (see above) made payable to Idaho Department of Insurance.
- Variable line will require FINRA CRD number for verification of a securities license. Firm must have active registrations on FINRA site.
- Independent Adjuster license use NAIC Uniform Application business entity and write IND ADJUSTER across top front of application and fee of $80.
- Public Adjuster license use NAIC Uniform Application for business entity and write PUBLIC ADJUSTER across top front of application and attach $20,000 bond or Irrevocable Letter of Credit and fee of $80. See Public Adjuster page on DOI website for more information regarding this license type.
- Surplus Line: Idaho does not license AGENCIES for Surplus Lines. Agency must carry the P&C license and have registered producer carrying the Surplus Lines license and P&C producer license. See Surplus Lines page on DOI website for more information regarding this license type.
- Bail Bond agency please attach bond and appointment from Surety Company
- MGA: please request MGA application packet from licensing. See below for contact information.
- Reinsurance Intermediary please consult Chapter 51 under LAWS/RULES/BULLETINS on our website. If non - resident and your state laws are substantially similar to Idaho 's and you are licensed in your state as a Reinsurance Intermediary, you may be exempt from licensing in this state. Residents, please contact the department for a licensing packet.
